Examining the FDA: The Government Accountability Office checked out the FDA, and found them wanting when it comes to drug safety. This is perhaps the most worrisome finding:

The GAO also recommended that Congress consider expanding the FDA's authority to require drugmakers to conduct postmarket safety studies. The agency sometimes asks for such studies as a condition of approval, but from 1991 to 2003, only a quarter were completed, the report said.

Sorry, but 12 years should be long enough to do a post-market safety study.
