Saw something very similar just the other day in the ER. A displaced gentleman living alone with a grossly enlarged liver (multiple mets). LFTs were only slightly elevated--I guess because he was near failure. SBP was in the 140-160 range. He was there for pain control and I didn't want to send him out on a new med along with powerful narcotics. He had psych issues and was a very poor historian. After getting some relief his SBP was in the 130s and DBP 80s-90s. I had him f/u with a new PCP/Onc for HTN control. Still not sure if that was the correct call.
