Saturday, April 07, 2007According to the study, about 1.16 million preventable patient safety lapses occurred in 40.6 million hospitalizations of Medicare beneficiaries from 2003 through 2005, compared with 1.24 million safety lapses in nearly 40 million hospitalizations from 2002 through 2004. However, the report found that patient safety problems worsened between 2003 and 2005 by 2.03 incidents per 1,000 hospitalizations. According to the report, 247,662 deaths could have been prevented during those years if hospitals had made fewer medical mistakes. New silver dimes and quarters were last produced for circulation in 1964. Half dollars were reduced after 1964 from 90% silver to 40%. In 1971, the use of silver was discontinued for circulating half dollars. While older silver coins continue to be legal for use, most were bought up by 1979 in the Hunt Brothers bubble. |
